Checkout History Request Form

As we transition to SCLENDS on November 10, your checkout history from our current system will not transfer.

If you would like a copy of your checkout history from the past three years, you can submit the below online request form by Monday, November 3rd. We will send your history to your provided email in a spreadsheet format. 

Please note: We will do our best to fulfill all requests, but due to potential high demand and time constraints, we may not be able to complete every request.
