The library has received a limited amount of the Federal 1040 booklets and forms.

The State of Indiana provided a very limited amount of the IT-40 forms.

Please bring the following to your appointment:
  • Picture ID and Social Security cards for everyone listed on the return
  • W-2’s and 1099’s from your employers
  • Other 1099’s for interest, dividends, state tax refund or other income
  • 1098 for education credit plus detailed financial statement from the school
  • Bank account number and routing number for direct deposit
  • Childcare information including Provider ID and actual amounts paid
  • Summarized list of other income/expenses for those who itemize or are self employed
  • Property Tax Paid on residents or rental information 
  • Refund Delay for the Earned Income Tax Credit and Child Tax Credit until February so the IRS can check to see that each child is only on one tax return
AARP Free In-Person Tax Preparation Service

AARP Foundation Tax-aide offers free tax preparation to low and moderate income taxpayers, especially those 50 or older or can't afford paid tax preperation. It is offered in conjunction with the IRS. This service begins in February and ends in April.

When you come, please bring the following:
  • Copy of last year’s tax return(s)
  • W-2 forms from each employer
  • Unemployment compensation statements
  • SSA-1099 form if you were paid Social Security benefits
  • All 1099 forms (1099-INT, 1099-DIV, 1099-B, etc.) showing interest and/or dividends and documentation showing original purchase price of sold assets
  • 1099-MISC showing any miscellaneous income
  • 1099-R form if you received a pension or annuity
  • All forms indicating federal income tax paid
  • Dependent care provider information (name, employer ID, Social Security number)
  • All receipts or canceled checks if itemizing deductions
  • Social Security cards or other official documentation for yourself and all dependents
