To receive a resident library card, a person is required to fill out a registration application. The following information is required: name, address, telephone number, plus proof of residency or property ownership in Washington Township. Children under 16 will need a parent or guardian to sign for borrowing privileges. Proof of Residency Acceptable Forms 1. Indiana Driver's License, or 2. Indiana state identification card, or 3. A middle/junior high or high school photo identification card for those (16) and over who do not have a driver's license. Or, in lieu of photo identification from the State of Indiana the following may be used 1. Other photo identification plus a Washington Twp. address utility bill, or 2. Other photo identification plus a Washington Twp.
